THe Literals #1 (Pt 3 of the Fables Crossover)

Another okay issue, but some questions.
1) Who is the person wearing the straitjacket that keeps popping up in every page with Kevin?
2) Has it been revealed who exactly Kevin is? What his relantionship is with the other Fables? i.e Mr. Revise keeps calling him father.
It seems as though Kevin's complete history might've been revealed in an earlier issue of Jack of Fables but not having gotten them, I wouldn't know.
3) Is the pen he writes with the source of his power?
Any help would be greatly appreciated.
